Space Maintainer for Kids: Types, Fitting, and Care
Dr. Muhammad Rahim, DMDOctober 7, 202610 min read
A space maintainer for kids is a small dental appliance that holds open the gap left when a baby tooth comes out years too early. Without it, nearby teeth can slowly drift into the space and crowd the adult tooth that's still growing underneath.
But not every early gap needs one. This guide explains when a space maintainer helps, the main types, how it's placed, and how to care for it. You can also browse all of our dental services for the rest of the family.
Did your child lose a baby tooth early?
Dr. Muhammad Rahim and Dr. Jasper Ainslie can check the space, take a quick look at the adult tooth underneath, and tell you whether your child needs a space maintainer.
A space maintainer for kids is a custom appliance, usually a thin metal wire attached to a nearby tooth, that keeps a gap open until the adult tooth is ready to come in. It's used when a baby tooth is lost early from decay, an extraction, or an injury, and the adult tooth is still years away.

Frequently asked questions
Usually only for a few days. Most kids notice the wire with their tongue at first and may chew a little carefully, but they adjust quickly. A 2025 review noted that space maintainers can actually support normal chewing and speech by keeping the teeth in their proper places.
Many dental plans cover space maintainers for children, but coverage depends on your plan and your child's age. Ask our front desk to check your benefits before treatment, and we'll give you a written estimate so there are no surprises.
Save the appliance if you can, keep your child from chewing on that side, and call the office. A loose or broken maintainer can often be recemented or repaired. Don't try to push it back in yourself, since a poor fit can irritate the gums or the tooth.
Sometimes. Some removable designs include a small artificial tooth for appearance, though they're less common for front teeth. Whether one makes sense depends on your child's age, how soon the adult tooth is due, and whether the gap affects eating or speech.
No. A space maintainer simply holds a gap open and doesn't move teeth. Braces actively straighten teeth and correct the bite. Some children with a space maintainer never need braces, while others may still benefit from orthodontic care later.
Baby molars usually fall out around ages 9 to 12, so a child who loses one at 5 may wear a maintainer for years.
How long
Until the adult tooth starts to come through, then it's removed.
Think of it as a placeholder. Baby teeth guide the adult teeth into position, so when one leaves early, something else has to keep its spot.
Why Does Losing a Baby Tooth Too Early Matter?
When a baby tooth comes out too early, the teeth on either side can tip or drift into the empty space within months. That shrinks the room for the adult tooth underneath, which may then come in crooked, blocked, or crowded. Early loss of back molars is the most common concern.
An early gap can start to close within months if neighboring teeth drift.

The jaw matters. A 2026 meta-analysis in BMC Oral Health by El-Motayam and colleagues found that losing a first baby molar early led to about 0.52 mm of space loss in the upper jaw and 1.78 mm in the lower jaw. The authors concluded that space maintainers are indicated after lower extractions, while upper cases may not routinely need one.
The change can start fast. A 2025 systematic review by Joshi and colleagues found significant space loss on the extraction side just six months after an upper first baby molar was removed.
Most early tooth loss comes from a few common causes:
Deep decay that leaves a baby tooth too damaged to save. CDC survey data show untreated cavities in 8.8% of US children ages 2 to 5 and 15.3% of those ages 6 to 11.
Falls and sports injuries that knock out or break a tooth.
Infection or an abscess that makes removing the tooth the safest choice.
Whatever the cause, the sooner a dentist checks the space, the more options you'll have.
Does Every Lost Baby Tooth Need a Space Maintainer?
No. Whether a child needs a space maintainer depends on which tooth was lost, which jaw it was in, the child's age, how soon the adult tooth is expected, and whether the teeth are already crowded. A dentist usually checks an X-ray to see how close the adult tooth is before deciding.
Researchers point to the same factors. The 2025 Joshi review listed time since the extraction, crowding, the child's age, facial pattern, and how the back teeth meet as things that shape the plan.
Situation
What usually happens
Lower baby molar lost years before the adult tooth is due
A space maintainer is often recommended.
Upper baby molar lost early
The space may be watched closely, or a maintainer used, depending on the exam.
Adult tooth already close to coming through
Often no appliance, just monitoring.
Baby tooth lost only months before it would fall out
Usually no space maintainer is needed.
Does my child need one?
Was a back baby tooth lost years before it would normally fall out?
Yes: Ask about a space maintainer soon, before the gap starts closing.
No ↓
Does the X-ray show the adult tooth is still deep in the bone?
Yes: A space maintainer is more likely to help.
No ↓
Is the adult tooth already starting to come through?
Yes: Monitoring may be all that's needed.
No ↓
Not sure? A quick exam and X-ray give you a clear answer.
What Types of Space Maintainers Are There?
Space maintainers come in two main groups: fixed ones that are cemented to a tooth, and removable ones that your child can take out. Fixed designs like the band and loop or the lingual arch are most common in younger kids because they stay put and don't depend on remembering to wear them.
Type
How it works
Best suited for
Band and loop
A metal band around one tooth with a wire loop reaching across the gap
A single missing back tooth
Lingual arch
A wire behind the lower front teeth connecting molars on both sides
Several missing lower back teeth
Removable appliance
An acrylic plate, sometimes with a replacement tooth, that clips in
Older kids who can wear it reliably
A 2025 systematic review in Dentistry Journal by Casaña-Ruiz and colleagues looked at 11 studies with 479 children. Fixed designs like the band and loop and lingual arch were preferred for long-term stability and lower risk of loss, while removable ones required more cooperation from the child.
The same review noted that space maintainers help prevent neighboring teeth from drifting and may reduce the need for more involved orthodontic treatment later.
How Is a Space Maintainer Placed?
Placing a space maintainer usually takes two short visits. At the first, the dentist fits a band on a nearby tooth and takes a mold or digital scan. At the second, the custom appliance is cemented in place and checked for fit. There's usually nothing to repair, so the visits are quick.
A band and loop holds the space for one missing back tooth.
Exam and X-ray to check the space and the adult tooth underneath.
Fitting a small metal band on a neighboring tooth.
Taking a mold or scan so the appliance can be made to fit.
Cementing the finished space maintainer in place.
Checking the bite and showing your child how to keep it clean.
Most kids feel some pressure when the band goes on, and the new wire can feel odd against the tongue for a few days. That usually fades quickly as they get used to it.
Timing tip
Because space can start closing within months, ask about a space maintainer soon after a baby tooth is lost early, rather than waiting for the next routine checkup.
How Do You Care for a Space Maintainer?
Caring for a space maintainer means keeping it clean, avoiding foods that can bend or loosen it, and not poking it with fingers or the tongue. Regular checkups let the dentist make sure it's still fitting well and that the adult tooth is on its way.
Careful brushing around the band keeps the area healthy.
Checkups matter here. According to the ADA, regular dental visits can catch 80% of oral health issues before they become serious, and a loose band is much easier to fix early.

Myth
Once it's in, a space maintainer can be forgotten about until the adult tooth arrives.
Fact
It needs regular checks. Bands can loosen and wires can bend, and the dentist needs to time its removal as the adult tooth comes through.
How Long Does a Space Maintainer Stay In?
A space maintainer usually stays in until the adult tooth starts to come through, which can be anywhere from months to several years depending on the child's age when the baby tooth was lost. Once the new tooth begins to appear, the dentist removes the appliance in a quick visit.
According to the ADA's tooth development charts, baby first molars usually fall out around ages 9 to 11 and baby second molars around 10 to 12, with the adult premolars arriving around ages 10 to 12. A child who loses a baby molar at 5 could need a maintainer for several years.
Tooth milestones timeline
Day of loss
The baby tooth comes out early from decay, extraction, or injury.
Within weeks
The dentist checks the space and the adult tooth on an X-ray.
Soon after
If needed, the space maintainer is fitted and cemented.
Every checkup
Fit, cleanliness, and the adult tooth's progress are reviewed.

The appliance is removed so the new tooth can come in.
The evidence for how much space maintainers prevent crowding long term is still limited. A 2022 review in the Saudi Dental Journal by Khalaf and colleagues called the current evidence very low and urged better studies. That's why the AAPD guidance on the developing dentition focuses on careful case-by-case decisions.
How Do We Handle Early Tooth Loss at Peterborough Family Dental?
We check the gap, take an X-ray when needed, and give you a straight answer on whether a space maintainer will actually help. If it will, we place it in our Peterborough office and follow it at every checkup. If it won't, we'll explain why and simply keep an eye on the space.
Our space maintainers are part of our interceptive care for kids, alongside habit appliances and early checks that guide growing smiles. Parents are welcome in the room, and we explain each step before we start. Regular care pays off over time, too: according to the Journal of Dental Research, people who visit a dentist regularly are 60% less likely to lose teeth.
Holding space for one tooth is a different question from how your child's whole bite and jaws are developing. That bigger picture is covered at an early orthodontic evaluation, usually around age 7.
Early tooth loss is one of many milestones we watch as kids grow. Our guide to kids' dental care by age shows what to expect at each stage.
An early gap is easy to overlook because it usually causes no discomfort. A quick visit soon after the tooth comes out is the simplest way to protect the adult tooth that's still on its way.
