A little preparation makes your first appointment smooth and unhurried. Here is exactly what to bring and how the paperwork works.
A driver's license or other government-issued ID for your file.
If you have dental insurance, bring the card so we can verify benefits and file claims for you. No insurance? Ask about our membership plan.
Include prescriptions, over-the-counter medicines, and supplements, with doses if you know them. It matters more to safe dental care than most people expect.
If you have recent X-rays from another office, ask them to send the images to us before your visit. It can save you time and repeat imaging. If you cannot get them, do not worry; we take our own as needed.
When you schedule, we send your new patient forms ahead of your visit so you can complete them at home, at your own pace, with your medication bottles and insurance card in front of you.
Prefer paper, or run into a confusing question? Come a few minutes early and our front desk will walk through everything with you. Nobody gets handed a clipboard and left to figure it out alone.
