Before age four or so, usually not, most kids stop on their own. If the habit continues as adult teeth start arriving, it can push teeth and reshape the bite. We start with kind, practical coaching for home; a habit appliance is the gentle backup when a determined thumb wins.

A small, custom metal or acrylic holder that keeps the gap open after a baby tooth is lost early. Baby teeth are nature's space maintainers, when one goes too soon, neighbors drift into the gap and the adult tooth underneath runs out of room. Holding the space now often prevents crowding and extractions later.
